High School Transfer and Foreign Exchange Students

If you are a high school STUDENT TRANSFER or a high school FOREIGN EXCHANGE STUDENT, it is REQUIREDthat you complete the corresponding form below before proceeding with registration

Sports Physicals

Minnetonka High School and Middle Schools are required to follow the Minnesota State High School League (MSHSL) rules that do require sports physicals for all MSHSL sanctioned sports. Minnetonka High School also requires sports physicals be on file for: Cheerleading, Marching Band and Fall Performance Dance.

The sports physical must clear the student for participation in sports and be dated within three years and remain current throughout their sport/activity season. Without this up-to-date clearance physical, you will not be able to register your student. (Please note: Well child physicals often do not clear your child for sports, so be sure to ask for a sports physical to be done).

How to find out if your student has an up-to-date sports physical, must be current within 3 years on record with Activities office

  1. First, please check in your student’s SSO/Student Skyward. Under their Health Information, choose "Physical Exam" to see what date is on file for your student. If there is no date showing, then we do not have one on file for your student.
  2. If you have difficulty in locating the above, please contact MHS activities admin:  gwynn.pletsch@minnetonkaschools.org

Sports Physicals may be submitted by email, FAX or in person.   Please scan and email your student's sports physical to their school's Student Activities Assistant, or FAX it to their school's Activities Office at their FAX number below, or drop it off in person.

Student plans (IEP, 504 Plans, Health Plans) are not automatically shared with the coach/advisor. Parents requesting that information to be shared with the coach/advisor may email  ted.schultz@minnetonkaschool.org or your student's case manager/plan manager.
