New Patients
As a new patient of Plano Orthopedic & Sports Medicine Center you will be asked to complete some required medical history and registration forms to bring to your initial appointment. If this is not possible, please plan to arrive 15-20 minutes early to allow time to complete the forms provided in our office. Please understand these forms are to ensure we have accurate information on you as a patient, including but not limited to, health insurance, medical history, current medications and history of your current injury or condition. All of this information allows our physicians to provide the best care possible by knowing your medical needs.
- Fill out new patient registration forms linked to the physician you are seeing. Click on physicians name below. Fill out, print and bring to your office appointment.
- An email will be sent to you with a login to the patient portal link to complete the “Health History Form” . This email should be received within 24 hours of making your initial appointment. This gives you access to your medical information via our portal as well.

X-Ray Pregnancy Consent Form: Please bring the signed form below if you are a woman in order to comply with the consent of x-ray and the affects on pregnancy.
Prescription Medication: Hydrocodone Rules:
Effective October 6, 2014 you must have a written prescription
to give to your pharmacist for Hydrocodone medications. To learn more about this please review the form below.
